Overview:
We're seeking an experienced Advanced Nurse Practitioner (ANP) to join a dynamic GP practice in Chichester. You'll work autonomously within a total triage model, managing a mix of telephone, online, and face-to-face consultations. The role includes independent prescribing and regular home visits to care homes and private residences.

Key Responsibilities:

• Assess, diagnose & treat undifferentiated and chronic conditions
• Conduct daily home visits
• Prescribe and review medications (V300 required)
• Collaborate closely with GPs and MDTs
• Support urgent care and manage complex cases
• Participate in audits, safeguarding, and quality initiatives

Requirements:
✅ NMC Registered Nurse
✅ MSc/Postgrad in Advanced Clinical Practice (or equivalent)
✅ V300 Independent Prescriber
➕ SystmOne experience (desirable)
