GP Surgery - Southminster, Essex

About the Role

We are seeking an experienced and motivated Advanced Nurse Practitioner (ANP) to join our dynamic multi-disciplinary team. Working both autonomously and collaboratively, you will provide high-quality, patient-centred care across the surgery.

In this role, you will use your advanced clinical skills, critical thinking, and independent decision-making to assess, diagnose, treat, and evaluate patients presenting with minor illnesses and ongoing clinical needs. You will be a key clinical resource within the surgery, supporting the delivery of safe, effective, and compassionate care.

Key Responsibilities

  • Act as an expert practitioner, demonstrating advanced clinical competence beyond conventional nursing roles.
  • Provide autonomous, patient-centred clinical care across a range of settings.
  • Independently prescribe within current guidelines and review patient medications in line with formulary requirements.
  • Deliver face-to-face, telephone, video and e-consult appointments, and undertake home visits where appropriate.
  • Manage complete episodes of care, including assessment, diagnosis, treatment, referral and discharge.
  • Consult with patients of all ages, including children, using sound clinical judgement.
  • Work effectively within the multi-disciplinary team, using the expertise of colleagues to support best patient outcomes.
  • Maintain clear, accurate and contemporaneous clinical records to agreed standards.
  • Contribute to clinical targets including QOF, QIS and Enhanced Services.
  • Recommend and interpret appropriate diagnostic tests, investigations and reports.

Person Specification

Essential Qualifications:

  • Registered Nurse (NMC)
  • Postgraduate Diploma or Degree in Advanced Practice
  • Qualified Independent Nurse Prescriber (NMC)
  • Evidence of meeting NMC revalidation requirements

Essential Experience & Skills:

  • Meets recognised ANP standards at advanced level
  • Ability to record accurate clinical notes
  • Works confidently within own clinical scope and identifies when to refer
  • Demonstrated leadership and management experience
  • Broad primary care experience, including clinical triage
  • Strong problem-solving and analytical skills
  • Knowledge of health promotion approaches
  • Understanding of PGDs and related policy
  • Professional, flexible, empathetic and highly motivated
  • Able to work effectively under pressure and in distressing situations
  • Demonstrates integrity, loyalty, and excellent communication skills
