Clinical Pharmacist - GP Surgery
Location: Wiltshire (Melksham - Spa and Giffords Surgeries)
Salary: up to £55,000 per annum
Hours: Part Time/Full-time, up to 37.5 hours/week (job share considered)
Contract: Permanent

Job Summary
Are you an experienced Clinical Pharmacist passionate about enhancing pharmacy services in primary care? Healthii People is proud to be recruiting on behalf of a thriving Primary Care Network (PCN) in the Wiltshire area. This is a unique opportunity to join an established team of pharmacists and pharmacy technicians working across two friendly, modern GP surgeries based in Melksham.
This full-time role (job share considered) involves supporting the delivery of a proactive, patient-focused pharmacy service for over 48,000 patients. The successful candidate will join a collaborative team committed to innovation and high-quality care, working closely with community pharmacy colleagues and contributing to service development initiatives like Pharmacy First and advanced medication reviews.
Main Duties and Responsibilities
- Conduct Structured Medication Reviews (SMRs), including for care home residents and patients with complex needs.
- Support the management of long-term conditions such as hypertension, diabetes, and asthma.
- Improve medicines optimisation and safety during transfers of care.
- Contribute to the repeat prescribing process and handle acute medication requests.
- Actively support clinical audits and quality improvement projects.
- Complete the CPPE Clinical Pharmacist Pathway if not already done.
- Engage in personal development and contribute to the ongoing evolution of clinical pharmacy within the PCN.
About You - Person Specification
Essential:
- Registered Pharmacist with the General Pharmaceutical Council (GPhC).
- Minimum two year's post-registration experience.
- Strong clinical background with a good understanding of acute and chronic conditions.
- Committed to improving patient outcomes and capable of working both independently and as part of a team.
- Willingness to complete required development pathways and work flexibly as needed.
Desirable:
- Independent Prescriber qualification.
- Experience working in general practice or within a PCN setting.
- Familiarity with NHS primary care structures and medication safety systems.
- Membership of the Royal Pharmaceutical Society and/or relevant postgraduate qualifications.
