Clinical Pharmacist - Primary Care Network (Part-Time / Full-Time)

Overview

A progressive Primary Care Network in Camden is seeking a Clinical Pharmacist to join its multidisciplinary team. This is a part-time or full-time position supporting GP practices to deliver high-quality, patient-focused care.

Key Responsibilities

• Deliver face-to-face, telephone, and home visit consultations
• Conduct clinical medication reviews and SMRs
• Manage acute and repeat prescribing processes
• Provide medicines optimisation support and contribute to QOF and PCN DES targets
• Perform medication reconciliation post-discharge and at care transitions
• Support antimicrobial stewardship and quality improvement initiatives
• Advise patients on medication use, lifestyle, and self-management
• Collaborate with GPs, nurses, pharmacy teams, and wider healthcare partners

Requirements

• MPharm (or equivalent) and GPhC registration
• Clinical diploma and ongoing CPD
• Independent prescriber (or willingness to train)
• Minimum 3 years' post-registration experience in the UK
• Strong communication, organisation, and clinical skills
• Ability to work autonomously and within a multidisciplinary team

Desirable

• Primary care experience
• Knowledge of EMIS, Accurx, Docman
• Experience with audits, prescribing data, and quality improvement
