The Role

As a valued member of the nursing team, you will:

• Provide evidence-based care for patients with chronic conditions
• Deliver preventative healthcare and promote healthy lifestyle changes
• Carry out key clinical procedures, including cervical cytology, immunisations, vaccinations, ear care, and minor surgery
• Support service improvement initiatives and contribute to clinical audits
• Mentor and supervise colleagues as part of a collaborative primary care team

What We're Looking For

• Registered Nurse with minimum 2 years post-registration experience
• At least 1 year in General Practice and 1 year community nursing experience
• Proven experience in nurse-led chronic disease management
• Excellent clinical, communication, and IT skills
• A proactive and flexible team player who shares our CARE values - Commitment, Accountability, Respect, and Excellence
