Lead Practice Nurse - Independent Prescriber
Location: Hemel Hempstead
Salary: Up to £50,000 per annum

Healthii People are recruiting on behalf of a well-established GP practice in Hemel Hempstead seeking a highly experienced Lead Practice Nurse (Independent Prescriber) to join their dynamic and supportive clinical team.
Role Overview:
As Lead Practice Nurse, you will work autonomously managing a clinical caseload of patients presenting with undiagnosed acute conditions and routine patients with long-term conditions (LTCs) such as diabetes, asthma, COPD, and hypertension. You will conduct comprehensive assessments, diagnose, treat, prescribe, order and interpret investigations, and refer patients as appropriate.
You will provide clinical leadership and mentor-ship within the nursing team, ensuring delivery of high-quality, patient-centred care aligned with NHS policies, NICE guidelines, and professional standards. You will support health promotion, chronic disease management, and preventative care initiatives within the practice.
Key Responsibilities:
- Independently assess, diagnose, and manage patients presenting with undiagnosed or chronic conditions.
- Prescribe medications safely and effectively within your scope as an independent prescriber.
- Lead and develop specialist primary care services, including LTC management programmes.
- Support and mentor nursing colleagues and health-care assistants, fostering a collaborative team environment.
- Participate in clinical audits, quality improvement projects, and compliance with Care Quality Commission (CQC) standards.
- Deliver opportunistic and targeted health promotion and disease prevention activities.
- Work collaboratively within a multidisciplinary team including pharmacists, physiotherapists, midwives, mental health workers, and paramedics as part of the local Primary Care Network.
About the Practice:
Our practice serves a diverse population of approximately 17,500 patients. The nursing team consists of three practice nurses and two healthcare assistants, supported by an expanding multidisciplinary clinical team. Person Specification:
- Registered Nurse with NMC registration and independent non-medical prescriber qualification
- Proven experience in primary care or A&E managing acute and chronic conditions
