Practice Nurse - Specialist Interest in Diabetes or Respiratory

Lydney, Gloucestershire

Contract & Salary

  • Part-time or full-time
  • Up to £47,000 per annum
  • Closing date: Friday, 2nd January 2026

Role Summary

Deliver high-quality primary care, focusing on long-term condition management in Diabetes or Respiratory. Support self-management, preventative care, and acute/long-term condition consultations.

Key Responsibilities

  • Conduct face-to-face, telephone, video, and e-consultations
  • Manage acute and long-term conditions, provide vaccinations, screening, and health promotion
  • Maintain accurate IT-based patient records
  • Prescribe in line with guidelines (if qualified)
  • Contribute to audits, quality improvement, and patient safety initiatives
  • Support and cover for other nurses as required

Person Specification

Essential:

  • Registered Adult/General Nurse (NMC) with current registration
  • General practice experience and degree-level specialist qualification in long-term condition
  • Excellent communication, IT, teamwork, and autonomous decision-making skills
  • Polite, empathetic, flexible, motivated

Desirable:

  • Independent prescriber or working towards
  • Understanding wider determinants of health
  • Driving licence and vehicle

Working Conditions

  • Frequent computer use, time-pressured environment
  • Exposure to body fluids and distressing situations
  • Uniform required, DBS check required

Reporting Lines

  • Clinically: Lead Nurse
  • Operationally: Practice Manager
