Practice Nurse
Part-Time/Full-Time: Can work 37.5 hours over 4 days
Salary: Up to £45,000 per annum

Job summary
This is an exciting opportunity to join a forward-thinking Practice, providing the highest standard of patient-centred health care.

We have a population of up to 10,000 patients and we lead the way for Primary Care in our area. Our Practice's QOF figures are consistently high and we provide scope for our Clinicians to develop special interests in Primary Care.

Main duties of the job
The successful candidate will manage patients with long-term conditions as part of a large clinical team including GPs, ACPs, Practice Nurses, Pharmacists, and Health Care Assistants.

About us
We are a supportive training practice with 5 GP partners, a Salaried GP, an ACP and 2 GP Registrars. Our administrative and clinical teams provide excellent support. We have approximately 9,700 patients and operate from purpose-built premises. We are part of a local Primary Care Network, use TPP SystmOne, and are high QOF achievers with a Good CQC rating.

We offer patient-centred care including chronic disease management, antenatal/postnatal care, minor surgery, vaccinations, sexual health services, and well-person checks.

Job responsibilities

• Manage chronic diseases (e.g., diabetes, respiratory, cardiovascular)
• Deliver health promotion, screening, vaccinations, cervical cytology, travel and sexual health services
• Work within a multi-disciplinary team and the Primary Care Network
• Mentor colleagues and commit to ongoing professional development
• Use TPP SystmOne for patient records

Essential Qualifications

• Registered Nurse (NMC) with General Practice experience
• Skills in long-term condition management, immunisations, vaccinations, cervical screening, and complex wound care
• General Practice Nursing responsibilities
