About the Role

Healthii People is delighted to be recruiting on behalf of a welcoming and forward-thinking GP practice in Huddersfield for an experienced Advanced Nurse Practitioner (ANP).

This is an exciting opportunity to join a supportive, multidisciplinary team dedicated to delivering exceptional, patient-centred care. You will work autonomously to assess, diagnose, treat, and manage patients with a wide range of acute and chronic conditions through face-to-face, telephone, and online consultations.

Key Responsibilities

• Provide expert assessment, diagnosis, and management of patients with acute and chronic conditions.
• Prescribe medications, order and interpret diagnostic tests, and make appropriate referrals.
• Lead on health promotion, disease prevention, and patient education.
• Support service development, clinical leadership, and quality improvement initiatives.
• Maintain accurate and compliant clinical records using appropriate coding systems.
• Contribute to practice targets (e.g., QOF) and uphold safeguarding responsibilities for adults and children.

About You

Essential Qualifications and Experience:

• Registered Nurse (NMC)
• Master's degree (or equivalent, meeting RCN Advanced Level Nursing Practice requirements)
• Independent Nurse Prescriber (NMC registered)
• Experience in chronic disease management, prescribing, and clinical leadership
• Strong understanding of evidence-based practice, safeguarding, and clinical governance

Desirable:

• Knowledge of Primary Care Networks (PCNs) and wider NHS structures
• Experience with QOF and enhanced services
• Awareness of public health priorities at local and national levels
