Location: Leighton Buzzard, Bedfordshire (Primary Care Network Setting)

Contract Type: Permanent
Working Hours: Up to 37.5 hours per week
Salary: Up to £55,000 per annum (dependent on experience)

About the Role
Healthii People are recruiting on behalf of a Primary Care Network for an experienced and motivated Clinical Pharmacist.
This is an exciting opportunity to join a forward-thinking PCN that is developing new services to improve patient care. You will work as part of a multi-disciplinary team, supporting medicines optimisation, long-term condition management, and improving prescribing quality across multiple GP practices.
We welcome applications from qualified pharmacists with primary care experience or those who are independent prescribers (training can be provided).

Key Responsibilities
- Conduct clinical medication reviews for patients with long-term conditions and complex polypharmacy.
- Support GPs, nurses, and practice staff with prescription and medication queries.
- Undertake care home and domiciliary medication reviews.
- Manage repeat prescriptions, acute requests, and medicines reconciliation following hospital discharge.
- Provide patient-facing clinics, telephone medicines support, and education on medicines use.
- Contribute to reducing unplanned hospital admissions by reviewing high-risk medicines.
- Lead on medicines safety, clinical audits, and implementation of national prescribing guidance.
- Provide training and support to the primary care team on therapeutics and medicines optimisation.
About the Primary Care Network
This PCN supports a large patient population across several GP practices in Leighton Buzzard, Bedfordshire. The team works collaboratively to deliver proactive and community-focused care to improve patient health outcomes.
Person Specification
Essential:
- GPhC registered pharmacist (MPharm or equivalent).
- Minimum of 2 years' post-qualification experience.
- Sound therapeutic and clinical knowledge.
- Excellent communication and IT skills.
- Ability to work both independently and as part of a multi-disciplinary team.
Desirable:
- Independent Prescriber qualification (or willingness to train).
- Experience in primary care or general practice.
- Postgraduate clinical pharmacy qualification.
- Full UK driving licence.
