Location: Bristol
Hours: Part Time (up to 30 hours)
Salary: Up to £58,000 per annum (pro rata)
Summary
Healthii People Agency is seeking an enthusiastic and motivated Paramedic or Advanced Nurse Practitioner to join a supportive GP practice team on a part-time basis (up to 30 hours per week). This role is ideal for an experienced clinician with an interest in frailty care, delivering high-quality assessment and management of patients within care home settings.
Key Responsibilities
- Work autonomously within professional boundaries to provide holistic, patient-centered care.
- Undertake weekly ward rounds, assessing new residents and managing both acute and chronic conditions.
- Develop and maintain personalized care plans and communicate effectively with patients, families, and the wider clinical team.
- Ensure completion of PCN DES requirements and contribute to medication reviews.
- Use evidence-based practice and comply with all relevant national guidelines and clinical governance standards.
- Support audits and initiatives to improve care quality, including QOF-related outcomes.
- Collaborate within a multidisciplinary team to optimise patient care.

Person Specification
Essential Experience
- Triage and initial patient assessment (face-to-face and telephone)
- Management of minor ailments and elderly/frailty patients in primary care
- Competency in autonomous clinical practice
- Up-to-date knowledge of NICE guidelines, safeguarding, and clinical governance
- Strong examination skills and evidence of CPD
Desirable
- Experience managing complex care needs
- Familiarity with EMIS clinical systems
Skills
- Excellent communication and interpersonal skills
- Ability to work independently and within a multidisciplinary team
- Strong time management and IT proficiency
- Access to own transport
Qualifications
Essential
- Registered with NMC or HCPC
- Independent Prescribing qualification
- MSc in Advanced Clinical Practice
Desirable
- Minor illness and chronic disease management qualifications
