Salary: Up to £60,000 per annum, depending on experience
Hours: Full-time or part-time considered

Are you an enthusiastic and clinically driven Clinical Pharmacist looking to make a meaningful impact in primary care? Healthii People are proud to represent a well-established and forward-thinking GP practice in Wiltshire, currently seeking a Clinical Pharmacist to join their expanding multidisciplinary team.

About the Role:

This is a patient-facing role with a strong focus on medicines optimisation and chronic disease management. You'll conduct structured clinical medication reviews, support repeat prescription systems, and help deliver safer, more effective prescribing practices. You'll work alongside GPs, nurses, paramedics, and other professionals to deliver integrated care for patients with complex needs, especially those managing long-term conditions or polypharmacy.

Key Responsibilities:

  • Conduct medication reviews and support medicines optimisation
  • Manage repeat prescriptions, acute requests, and medicine queries
  • Support long-term condition clinics (e.g., hypertension, asthma, diabetes)
  • Reconcile medicines post-discharge or care transfer
  • Respond to MHRA alerts and implement prescribing safety measures
  • Collaborate with wider clinical and administrative teams to meet QOF, IIF, and CQC standards

About the Practice:

  • Serving over 12,000 patients across two sites
  • High QOF achievement and strong CQC rating
  • Full administrative support, including workflow optimisation
  • Part of a proactive Primary Care Network with access to paramedics, social prescribers, FCPs, and more
  • Uses SystmOne, AccuRX, Cinapsis, and other integrated digital tools

What We're Looking For:

  • GPhC-registered Pharmacist (essential)
  • Primary care experience desirable but not essential
  • Strong communication skills and a patient-first mindset
  • Willingness to work towards independent prescribing and further clinical training
