Clinical Pharmacist (Primary Care)

Location: Rhyl, North Wales | Sector: Primary Care | Hours: Full-time | Contract Type: Permanent
Salary: Up to £55,000 per annum, dependent on experience

Are you a motivated and forward-thinking Clinical Pharmacist looking to make a real difference in patient care? We are offering an exciting opportunity to join a dynamic, multidisciplinary team in Rhyl, North Wales, focused on innovation, collaboration, and personalised healthcare.

This is a fantastic opportunity for an independent prescriber with a passion for chronic disease management, medicines optimisation, and mentorship.

💼 What You'll Be Doing

  • Lead and develop pharmacist-led chronic disease clinics
  • Conduct structured medication reviews and contribute to proactive care models like Year of Care
  • Work closely with GPs, nurses, and other healthcare professionals in an integrated team environment
  • Support and mentor pharmacy students, pre-registration technicians, and trainees
  • Engage in research and service development to help shape future clinical practice

What We're Looking For

  • MPharm degree (or equivalent)
  • Postgraduate Clinical Pharmacy Diploma or MSc (or equivalent experience)
  • General Pharmaceutical Council registration
  • Independent Prescriber (or working towards)
  • Strong collaborative skills and the ability to meet deadlines in a busy care setting

🌟 Desirable Skills

  • Experience in teaching or mentoring in clinical settings
  • Welsh language skills (desirable but not required - English speakers are equally welcome)
