The Role

• Provide evidence-based care for chronic conditions
• Support health promotion & preventative care
• Work within a multidisciplinary team to deliver high-quality patient care
• Contribute to service improvements & mentor colleagues

About You

✔ Minimum 1 year General Practice & 2 years post-registration experience
✔ Skills in chronic disease management, cervical cytology, immunisations, ear care, minor surgery
✔ Strong communication, leadership & IT skills
✔ Flexible, professional & patient-focused
