Practice Nurse (Maternity Cover)
Location: Ilminster, Somerset
Contract: 12-month fixed term, maternity cover
Hours: Part-time, 20 hours per week
Salary: Up to £45,000 per annum (pro rata, DOE)

About the Role

Healthii People Agency is recruiting on behalf of a leading primary care provider in Ilminster, Somerset for a Practice Nurse.

This is a fantastic opportunity for an experienced nurse to join a supportive GP practice team, delivering high-quality patient care and contributing to service development.

Main Duties

• Provide nursing care for patients with long-term and complex conditions.
• Use clinical systems for recalls, screening, and accurate data entry.
• Support the practice team with the implementation of new initiatives in patient care and chronic disease management.
• Contribute to the development of policies, protocols, and guidelines.
• Promote integrated services within primary care through multidisciplinary working.
• Assist with TEP reviews and effective care planning.

Person Specification

Essential:

• Registered first level nurse
• Evidence of ongoing CPD
• Minimum 2 years' recent experience working with patients with chronic conditions
• Skilled in management of long-term and complex needs
• Experience with audit and quality improvement
• Strong communication, negotiation, and conflict resolution skills

Desirable:

• Mentor/teaching qualification
• Clinical supervision training
• Knowledge of EMIS/SystemOne and Ardens
• Clinical examination skills (PACR)
• Non-Medical Prescribing (NMP)
• Change management experience
