Practice Nurse - GP Practice, Coventry

Salary: Up to £43,000 per annum
Hours: Part-time - up to 20 hours per week

Job Summary:
Join a friendly, community-focused GP practice to deliver high-quality nursing care. Manage patients with long-term conditions, support elderly and housebound patients, and lead services including infection control, chronic disease management, vaccinations, and health promotion.

Key Responsibilities:

• Provide routine and acute nursing care based on evidence-based practice (NICE, NSF).
• Manage chronic diseases, implement treatment plans, and run disease-specific clinics.
• Conduct well-woman/man clinics, vaccinations, travel medicine, and minor procedures.
• Support infection prevention and control initiatives, audits, and policy compliance.
• Maintain accurate clinical records and chronic disease registers.
• Provide patient education and guidance on medications.
• Mentor the nursing team, coordinate rotas, and participate in practice initiatives.
• Assist with safeguarding, mental health referrals, and care for vulnerable patients.

Experience & Skills:

• Essential: Primary care experience, chronic disease management, autonomous working, infection control, wound care, ECGs, venipuncture, immunisations, women's health, PGD use, accurate record-keeping.
• Desirable: Diabetes care, travel medicine, spirometry, clinical governance knowledge, health promotion strategies.

Qualifications:

• Registered Nurse (NMC) - essential.
• Mentor/teaching qualification, postgraduate diploma/degree in Chronic Disease Management, or family planning qualification - desirable.
